Please log in using the details sent to your email ID. You will be prompted to change your password on first login.
You will see list of ‘Area of work’ and activities in the form.
Please consider your pre-covid work schedule and insert the average number of hours you spend on each activity either on a daily/ weekly/ monthly/ quarterly/ yearly basis. Eg: 2, 3, 4.
You can even enter 0.5 hours for some activities, Eg: 0.5, 1.5, 2.5. However, entries like 1.3, 2.2, 4.1 are not allowed.
In case there are additional activities you want to add, please click on (+) button at the bottom right side of the page, put additional area of work and activity and click on ‘Save’.
Activities which you don’t perform please leave them ‘blank’.
Please keep saving your responses.
Click on ‘Submit’ once you have completed the survey.
Kindly be sure of your responses before submitting since post submission, the survey link will no longer be active for you.
